West Hills Health & Rehabilitation Center is a licensed skilled nursing facility located in the West San Fernando Valley. Our center provides 24-hour nursing care, rehabilitation services, and supportive care in a structured clinical environment.

Licensed nursing staff are on site around the clock to support residents’ medical, functional, and daily care needs in accordance with individualized care plans and physician orders.

We offer short-term rehabilitation and long-term care services, including physical, occupational, and speech therapy, as clinically appropriate. Therapy programs are developed based on individual assessments and coordinated by an interdisciplinary care team.

Transitional care services are available for individuals recovering after hospitalization, as well as long-term skilled nursing care for residents requiring ongoing support. Services are provided in accordance with regulatory requirements and clinical standards.

Residents also have access to amenities and activities designed to support comfort, engagement, and daily living. Availability of services and amenities may vary based on individual needs and care plans.
